Student General/Medical Release - These forms will only have to be completed once a year and will need to be notarized, but will eliminate the need to complete a form each time your child participates in an off-site event. Forms may be picked on the table in the Sanctuary, from the church office or by clicking HERE. Please contact Angela Burke for FREE notary service or should you have any questions or concerns.
